CIPTS

.

CIPTS is our patient based clinical information system that is used within hospital specialties to accommodate the detailed clinical data that is collected for each patient. The specialties that use CIPTS are:

 

· Dermatology

· Diabetes

· Endoscopy

· ENT

· Orthopaedic

· Plastic surgery

· Theatres

 

CIPTS is modular with many facilities being shared by all users and other facilities that are specific to a single specialty. Each user, on logging in, is presented with a menu that is targetted to the features that are both relevant and authorised for that user.

 

The major areas that are shared by most users are:

 

· Theatre management

· Letter writing

· Analysis

· Clinical lists

· Patient selection

 


Theatre management

Most of the facilities within the theatre management module are also available and used by the Endoscopy, ENT and Plastics modules. This enables them to set up theatre sessions, allocate patients to these sessions and then view a daily summary showing all of the sessions, each sized to show their length in time and each with a list of the patients allocated, with full drilldown to both an individual session and then to each patient within that sessions.

 

In addition, the Theatre module also holds details of transfers between theatres (to differentiate the “Actual” from the “Planned” utilisation). All of the anaesthetic and surgical timings are entered so that utilisation by anaesthetist may also be determined.

 

 

Letter writing

The letter writing facilities are used in most modules. These enable letters to be created from templates that identify fields and their positions within the letter together with any fixed text that appears unchanged for all letters using each template. Additional, individual free text can be inserted within any one letter and the whole text is stored within the database providing both rapid creation of individual letters plus immediate audit of all letters issued in respect of each patient. There is no limit to the number of templates that may be created. Letters may be directed to either the patient or GP, next of kin, consultant etc. Printing of letters is undertaken via Word. There are some installations of CIPTS users that use the system primarily for letter creation and one installation now has in excess of 230,000 letters.

 

 

Analysis

There are several analysis facilities. The most popular allows for a 3 level hierarchy to be specified eg admission months, consultants and operations which then generates an explorer style window that lists admission months from which one can be selected in order to show a sub-list of consultants for that month. From this a consultant can then be selected to create a further sub-list of operations undertaken by the selected consultant for the selected month and a separate list of all of the patients matching these selections is displayed with drilldown facilities to the full details of any one patient. This structure can be modified so that if, for example, it was reversed, the new hierarchy would then show operations first, then for a selected operation, the consultants that had performed this selected operation and then a list of the months in which this had happened. Lists of patients can be displayed at any of the 3 levels, graphs can also be displayed and printed and data may be downloaded to Excel. Any number of analyses may be set up and stored.

 

Another analysis facility enables users to define groups to which individual patients can then be linked in a multi multi relationship. These groups can then be included within the filters of other analyses.

 

 

Clinical lists

The diagnosis, treatments, operations, qualifiers, complications and drugs are using either Read codes, OPCS codes or lists that have been built up within a specialty. Dermatology use the BAD codes for their diagnoses. These lists can therefore vary between installations.

 

 

Patient selection

When the menu selection is made to enter or retrieve a single patient, the first field presented is the hospital number. This field can be used to reference a patient by their hospital number, name, NHS number or date of birth. Where more than one match is found, a list is displayed for selection. This field can also be used to retrieve the details of the last patient accessed by the user or a list of the most recent patients accessed from which a selection may be made. CIPTS accommodates multiple hospital numbers so that the entry of any one of the hospital numbers for a patient will retrieve that patient.

 

 

The facilities of CIPTS that are specific to a single specialty are:

Dermatology

The following areas of data are held for each patient:

 

· Patch tests, each with their own collection of allergens that hold indicators of results

· Diagnoses

· Investigations

· Treatments

· Drugs

 

 

Diabetes

For each clinic visit separate screens are used to enter any combination of the areas that have been examined. These are:

 

· General details (always entered)

· Diagnoses

· Pathology results

· Medications

· Glycaemic control

· Eye test results

· Renal

· Cardio

· Feet

· Neuropathy

· Urine

· Diet

· Other investigations

 

Detailed input forms and summary GP letters are generated from these details.

 

 

Endoscopy

For each procedure, the following data is collected:

 

· Referral details

· Endoscopists

· The findings and actions for each site examined with pathology references where applicable

· Results and follow up plans

· Photo chart (showing the positions at which photos were  taken)

 

Summary letters are generated and OPCS codes for the procedure are determined automatically.

 

A variety of patient lists may be displayed:

 

· All patients awaiting a date

· Patients booked but not yet admitted

· Patients unconfirmed

· Patients due to come in

· Patients with a range of admission  dates

 

All of these have drilldowns to individual patients. (These are also available to ENT & Plastics).

 

 

Orthopaedic

Where CIPTS is used as the main administration system (in a non NHS hospital), it uses the following features:

 

· Clinic session allocations

· Bed management and planning

· Invoicing (with download to Sage)

· Tracking of case notes, xrays etc

· Tracking of all prosthetics used

 

 

 

Waiting list management

CIPTS is also used for waiting list management using many features (deferrals, DNAs, suspensions etc) that are also available to other specialties Patients are scored using a formula developed by two consultants at Carmarthenshire NHS Trust where there are comprehensive analyses available for these waiting lists
